•a thorough examination of your teeth and gums•a full explanation of your treatment options•a written treatment plan including costs•information about NHS charges displayed in the waiting room•advice on how to keep your gums healthy•information about the practice and services available•a care and treatment summary if you decide to transfer to another dentist.•be able to make a complaint if you are not happy with your treatment and care.
You are responsible for
•giving at least 24 hours notice if you have to change or cancel an appointment. We will not charge you for appointments missed or cancelled at short notice (less than 24 hours) but if you do this twice we may not offer future NHS appointments.•Follow your dentist’s advice to prevent tooth decay and gum disease.•Pay your bill promptly.•declaring if you can claim for help with the cost of NHS treatment. If you claim help with NHS costs but are not entitled you are likely to receive a penalty charge of £100 from NHS England as well as your dental charges.•Treating our staff with courtesy and respect - we may refuse to treat patients who are abusive, fail to pay their bills or refuse to cooperate during treatment. In this case we will inform the patient and NHS England.
